The Toolbox
This 442 page document can help supported living agencies develop a toolbox of information, materials and resources for helping everyone learn about supported living. You can download each of the five sections (in pdf format) below:
  • Introduction. Ideas for setting up a staff training program including requirements from the SLS regulations.
  • Part One. History of residential and living arrangement services; introductory information on supported living services; and, the five principles or values of supported living services.
  • Part Two. Information Briefs on basic information about the service system for people with developmental disabilities.
  • Part Three. Tools to help staff do their work and implement the principles.
  • Part Four. Resources for the agency's staff training/resource library.
 
Bill and Dave's Corner
Named after Bill Silverberg and Dave Denniston, this place on the website will be used to post program designs, employment agreements and job descriptions for supported living services. It was started with posts from "Bill and Dave's excellent adventure" into individual vendorization. Bill manages his own supported living services and Dave provides some mentoring and support from time to time. Note: These documents are in Word or Excel for your easy downloading pleasure.
 
Some Words of Caution: Dave Denniston notes that most of the resources below were developed prior to Wage Order 15. So, you need to take that into consideration when looking at documents such as job descriptions and employment agreements.

  • SLS Agency Design is an outline to help you think about useful information to include in a service description for supported living services (V. Vining).

  • Employee Checklist is an example of the process used to hire and orient a new employee (Silverberg/Denniston).
  • Personal Assistance Agreement is the contractual agreement for the personal assistant and includes responsibilities, hours, wages, employment conditions, etc. (Silverberg/Denniston)
  • Timesheet is an example of an easy way for support staff to track their hours (Silverberg/Denniston).
  • Form W4 is used in conjunction with a standard W-4 for payroll purposes (Silverberg/Denniston).
  • Sample Schedule used by Sara Alewife was developed as an Excel spreadsheet (Silverberg/Denniston). In addition, here is the budget that matches that schedule.
 
General Information
Choosing a Supported Living Agency
  One of the different things about supported living is that the person who wants supported living and their family have the right to choose who the provider will be. You should carefully evaluate whether the provider's style of service matches what you are looking for in an agency. This list (adapted by Vickie Vining from some information developed by CHOICESS) will give you a good idea of the kinds of things that you might want to ask.
   
Individual Quality Check-Up
  for Supported Living Services was provided by Vickie Vining. It can be used by individuals, family members, service coordinators, or advocates who want to look at supported living services for an individual and see what's working well and what could be better.
   
Thoughts About My Life
  can be downloaded in Word format. This is a great piece developed by Karen Manning, Robin Reale, Michael Smull and Liz Obermeyer. It's a workbook that gives people a way to write stories about their lives. Those stories can be used to help facilitate planning meetings. They can also help family, friends and service agencies pay attention to what people say is important to them. Give it a try, you'll like what you see!
   
Getting to Know You
